Duly Health and Care Scholarship Fund

Duly Health and Care has established a scholarship program to assist its employees’ children who plan to continue their education in college or vocational school programs.

The program is administered by Scholarship America®, the nation’s largest designer and manager of scholarship, tuition assistance, and other education support programs for corporations, foundations, associations, and individuals. Eligibility for individual programs is determined at the sole discretion of the sponsor, and eligible applications are reviewed by Scholarship America’s evaluation team.

Applicants to the Duly Health and Care Scholarship Fund must be:

  • Dependent* children, age 26 and under, of part-time or full-time Duly Health and Care employees who have a minimum of one (1) year of employment with the company as of the application deadline.*Dependent children are defined as biological, step- or legally adopted children living in the employee’s household or primarily supported by the employee.
  • High school seniors or graduates who plan to enroll or students who are already enrolled in full-time undergraduate study at an accredited two-year or four-year college, university, or vocational-technical school for the entire upcoming academic year.

Children of c-suite, board members, leaders at vice president level and above, and doctors are ineligible.

Award Information:

  • Up to six (6) awards of $5,000 will be granted.
  • Awards may be renewed for up to three additional years or until a bachelor’s degree is earned, whichever occurs first.
  • Renewal is contingent upon satisfactory academic performance (maintaining a cumulative grade point average of 2.5 on a 4.0 scale or the equivalent) in a full-time course of study, continued employment of the student’s parent at Duly Health and Care, and continuation of the program by Duly Health and Care.

Students in two-year programs must transfer to a four-year program by the third year to be eligible for continued renewals.

Awards are for undergraduate study only.

Notification

Applicants will be notified in June. Not all applicants to the program will be selected as recipients. Students may reapply to the program each year they meet eligibility requirements.

Payment of Scholarships

Scholarship America processes scholarship payments on behalf of Duly Health and Care. Payment is made in one installment in early August.

Scholarship recipients are selected on the basis of:

  • Academic record
  • Demonstrated leadership and participation in school and community activities
  • Honors
  • Work experience
  • Statement of educational and career goals and objectives
  • Unusual personal or family circumstances
  • An online recommendation

At minimum, 50% or three (3) of the Duly Health and Care Scholarship Fund recipients will be in historically underserved minority populations assuming qualified applications are received.

Financial need is not considered.

Selection of recipients is made by Scholarship America. In no instance does any officer or employee of Duly Health and Care play a part in the selection. All applicants agree to accept the decision as final.

As part of your application, you must upload the following document:

  • A current, complete transcript of grades. Grade reports are not accepted. Unofficial or online transcripts must display student name, school name, grades and credit hours for each course and term in which each course was taken.

    NOTE: If you are providing SAT or ACT test scores, and those scores are not listed on your high school transcript, you will need to upload a copy of your test score report(s) separately. If you are uploading a college transcript, those scores are not required.

One online recommendation form must be submitted on your behalf no later than May 24, 2022 3:00 pm CT.
