The Medline Foundation Scholarship Program

The Medline Foundation has created a scholarship program to assist the dependent children of domestic Medline Industries, Inc. employees who are planning to further their education after high school. This program is administered by Scholarship America®, the nation’s largest designer and manager of scholarship, tuition assistance and other education support programs for corporations, foundations, associations, and individuals. Awards are granted without regard to race, color, creed, religion, sexual orientation, age, gender, disability, or national origin.

Applicants of The Medline Foundation Scholarship Program award must be:

  • Dependent* children, of full-time Medline Industries, Inc. domestic employees, who have a minimum of two years employment with the company as of the application deadline date.
  • High school seniors or graduates or current post-secondary undergraduates or graduate level students.
  • Planning to enroll full-time at an accredited two- or four-year college, university or vocational school for the entire upcoming academic year.
  • From a family with an adjusted gross income of $300,000 or less.

*Dependent children are defined as biological, step- or legally adopted children living in the employee’s household or primarily supported by the employee and qualify as a federal tax exemption to the employees.

If selected as a recipient, the student will receive:

  • An award up to $4,000 per school year. Up to ninety-five (95) new awards will be granted each year
  • Awards are renewable up to three consecutive years, on the basis of satisfactory academic performance maintaining a cumulative grade point average of 2.5 on a 4.0 scale
  • Awards are to be used for tuition, fees, books and supplies
  • Awards are for undergraduate, graduate study, and vocational-technical certificates

Scholarship recipients are selected on the basis of:

  • academic record
  • statement of goals and aspirations
  • unusual personal or family circumstances

Financial need will be considered. Recipients must demonstrate need.

Selection of recipients is made by Scholarship America. In no instance does any officer or employee of Medline play a part in the selection. All applicants agree to accept the decision as final.

Notification

Applicants (the student applicant, not the Medline employee parent) will be notified by email in June. Not all applicants to the program will be selected as recipients.

Payment of Scholarships

Scholarship America processes scholarship payments on behalf of The Medline Foundation. Scholarship checks will be made payable to the school and mailed to the student’s home address in early August.

As part of your application, you must upload the following documents:

  • A current, complete transcript of grades. Grade reports are not accepted. Unofficial or online transcripts must display student name, school name, grades and credit hours for each course and term in which each course was taken.
  • A copy of the first two pages of the prior year IRS form 1040, showing the student is claimed as a dependent on the Medline employee’s prior year tax return. Black out all social security numbers prior to uploading the 1040 information.

Your application is not complete unless all required documents are submitted electronically.
